Death certificates are recorded and filed in the City Clerk’s Office for those individuals who are pronounced deceased in the City of Yonkers. Death records are confidential records under New York State law. Death Certificates may only be obtained by the parents, siblings, children, and surviving spouse with proof of identification and proof of relationship or direct and tangible interest. All other individuals will require a Court Order.
NOTE: If your current last name does not match the last name on the death certificate (or on your birth certificate required to establish the link between siblings/children and the decedent), then please include/upload one of the following with your request:
- Marriage Certificate linking your current last name to your maiden/bachelor's (birth) name
- Final judgment of divorce linking your current last name to your maiden/bachelor's (birth) name
- Death Certificate linking your current last name to your maiden/bachelor's (birth) name
- Certified Court Order showing your legal name change from maiden/bachelor's (birth) name to current name

IN-PERSON REQUESTS:
Applicants must be at least 18 years old and must have valid photo identification. Death certificates may only be obtained by the surviving spouse, parents, children, or siblings of the deceased. Please note: if obtaining a parent's, or sibling's death certificate, then you must also produce your original birth certificate showing your parent's name.

The City Clerk’s Office is located in Room 102 at City Hall, 40 South Broadway, Yonkers, New York 10701. For driving directions, enter 87 Nepperhan Avenue, Yonkers, NY 10701 in your GPS device. It will take you directly to the municipal parking lot adjacent to City Hall.
