MISSION STATEMENT
The Yonkers NNORC seeks to transform the community into a vibrant environment where seniors can age comfortably in their homes. The Yonkers NNORC will provide a wide range of activities where seniors can make friends, develop new interests, further their areas of expertise and continue to grow as individuals. The NNORC will help seniors stay active both mentally and physically in the community that they love with a sense of purpose.
NNORC FUNDING
Westchester Jewish Community Services and the City of Yonkers, have been awarded a New York State Office for the Aging NNORC (Neighborhood Naturally Occurring Retirement Community) grant to help coordinate services and community resources to assist seniors maintain their quality of life and independence, and remain connected to their neighborhoods. By supporting aging in place, the NNORC not only helps seniors but contributes to the diversity and stability of their neighborhoods. The NNORC will include residential dwellings (single family homes and apartment buildings less than six stories) in Northeast Yonkers. It will encompass most of the area of Yonkers between Central Park Avenue and the Bronx River Parkway, as well as a portion of Tuckahoe Road.
PROGRAM SERVICES
RESOURCE SPECIALIST
• Case Management services to seniors 60+ and their families/caregivers, as needed to help them age safely in place
• Gain access to and coordinate appropriate services, benefits and entitlements
• Develop, implement and follow up of care plans
HEALTHCARE MANAGEMENT ASSISTANCE/MONITORING
• Health screening, consultation, and regular monitoring of blood pressure and other health indicators to seniors who have been identified to be at risk
• Assistance on health-related matters, such as the scheduling of appointments and contacts with pharmacies
• Home visits, phone calls and referrals as needed
INFORMATION AND ASSISTANCE
• Link seniors with appropriate services, program benefits, entitlements and other resources
• Assist with problems of daily living
• Offer short-term intervention to address a specific issue
• Provide information on a one-to-one basis
TRANSPORTATION
• Assistance with transportation
• Provide service for shopping, medical appointments and other supportive transportation
TELEPHONE REASSURANCE
• Provide supportive contact and monitoring on an on-going basis via regularly scheduled calls
• Provide supportive contact to seniors who are temporarily alone and have limited ability to leave their homes
• Helps ensure the health and safety of seniors
