Request for Inspection - READ ALL DIRECTIONS PRIOR TO SUBMITTING. INCOMPLETE APPLICATIONS NOT ACCEPTED. Use this form to request inspection for permit related inspections, Change of Tenant, Mobile Food Vehicles, tank work, etc. Fire protection system maintenance records may be required to close some permits and obtain operating permits. Refer to "Notice to Owners and Operators" in the section below.

Incident or Record Search - Request copies of Fire Department records and incident information. The link will re-direct to the FOIL request page through Yonkers Corporation Counsel. Select Fire Department in the drop down menu. Request information should be as specific as possible.

FIRE ESCAPE Inspection, Testing, & Maintenance Report - Required every 5 years as per IFC 1104.16.5.1.
MOBILE FOOD VEHICLES
As implemented by the 2020 New York State Fire Code, Mobile Food Vehicles that feature cooking operations that produce smoke and grease laden vapors are subject to Fire Code equipment and maintenance standards, Fire Department inspection, and require an Operating Permit. PERMIT REQUIREMENTS
All code required work and systems shall be performed under a valid permit. Consult with a design professional and licensed contractor for your project needs.
DESIGN PROFESSIONALS - Many plan submissions are denied due to vague information. Some of the items often missed are in the following list:
-Update notes to include the current NYS Code standard
-Include Alteration Level and provide a full code analysis. The following is an example of a more complete submission. Click to enlarge.

- Include manufacturer cut sheets on materials when needed for verification of specs. (Ex: To show product fire ratings, kitchen suppression systems, fire alarm devices, sprinkler heads and control valves, etc.)
- Provide firestop details including that apply to your project for membrane and through penetrations. Include manufacturer specific product and UL detail. Comparable alternative to be approved prior to installation.
Examples: Typical wiring and piping through penetrations, 4" and larger plastic pipe, medium to large voids accommodating various materials, combustible concealed spaces, electrical boxes in opposing wall sides, etc.
- Cleary define existing conditions, provide demo information, and show details pertaining to new work proposed (wall details, framing details, etc.) in addition to the aforementioned code analysis.
We follow NYS code so please verify all requirements for submissions. Below are more specific work types and their respective basic requirements, but always refer to code requirements for submissions.

NOTICE TO PROPERTY OWNERS/OPERATORS
It is YOUR responsibility to maintain your building in accordance with all code requirements. Consultation with a qualified and reputable design professional, contractor, servicing contractor, etc. will help you achieve this goal. Ignorance of the requirements is not acceptable.

ALERT!
**Removal of occupant use fire hoses is not allowed without prior approval from the Yonkers Fire Department, Fire Prevention Division. A design professional must provide code analysis showing compliance if hoses were to be removed. Upon review the Yonkers Fire Prevention Division will respond and if approved, removal can be performed. Until approval is obtained, systems must be maintained as designed.**
MAINTENANCE
The Fire Code requires records of all fire protection system inspections, tests and maintenance shall be maintained on the premises for a minimum of ten (10) years and shall be copied to the code enforcement official on request.
Pursuant to this requirement, beginning April 15, 2013, records of the system(s) listed below shall be copied to the Yonkers Fire Department. Inspection records shall be postmarked or hand delivered within ten (10) business days from the date of inspection. All records shall be addressed to:
